On behalf of the SGO Education Committee’s Chemotherapy/Targeted Therapies Subcommittee, this PDF resource provides important general guidelines for administering chemotherapy in patients on outpatient hemodialysis, not CRRT or peritoneal dialysis. 
 
Important Considerations:
  1. Is the proposed medication appropriate for a patient on hemodialysis?
  2. Is a dose modification needed?
  3. Does dialysis timing matter?
Note: Always consult and work with patient’s nephrology team and pharmacy.
